Telecom/Network Service Request Form

Instructions:

New Services

  1. Enter your information.
  2. Click Request and Budget for New Services. 
  3. Enter the end-user information or click the link if it is the same.
  4. Check the box(es) next to the service(s) you are requesting.
  5. Click the Calculate button.
  6. Enter your Account number(s).
  7. Print the form (for signature).
  8. Have your Department Head sign the printed form and fill in account information.
  9. Send or FAX the signed form to Finance (FAX: 404-752-1161).

Services Requested During the Budget Process

  1. Enter your information.
  2. Click Request Activation of Service Requested During the Budget Process.
  3. Enter the Request Number from the Telecom/Network Budget.
  4. Click Submit.  Your request will be submitted to the Help Desk.
